Conventional spine surgery refers to well-established surgical techniques used to treat spinal conditions where minimally invasive approaches may not be suitable. These procedures are typically considered when spinal cord or nerve compression is extensive, spinal stability is compromised, or spinal deformity requires broader correction.

This page provides an overview of conventional spine surgery and highlights procedures commonly performed as part of comprehensive spine care at Oxford Spine and Neurosurgery Centre.

When Conventional Spine Surgery May Be Considered

Conventional surgical approaches may be discussed when:
  • Symptoms significantly affect daily function or quality of life
  • Imaging shows significant spinal cord or nerve compression
  • Spinal instability or abnormal movement is present
  • Spinal deformity requires correction
  • Non-surgical or less invasive treatments have not provided sufficient relief
The decision to proceed with surgery is always based on careful clinical assessment, imaging correlation, and discussion of available options.

Role Alongside Minimally Invasive Spine Surgery

Conventional spine surgery is one of several surgical approaches used to manage spinal conditions. In selected patients with less extensive compression or instability, minimally invasive techniques may be considered as part of the treatment plan.

Surgical approach selection is guided by individual anatomy, symptom severity, spinal alignment, and overall treatment goals, with both conventional and minimally invasive options discussed where appropriate.

Key Conventional Spine Procedures

Anterior Cervical Discectomy and Fusion (ACDF)

Anterior Cervical Discectomy and Fusion is a commonly performed cervical spine surgery used to relieve pressure on spinal nerves or the spinal cord caused by disc herniation , degenerative changes, or bone spurs. The procedure is performed from the front of the neck and involves removing the affected disc and stabilising the spine by fusing adjacent vertebrae. ACDF may be considered for persistent neck or arm symptoms that have not improved with non-surgical treatment.

Posterior Cervical Decompression and Fusion (PCDF)

Posterior Cervical Decompression and Fusion is performed from the back of the neck and is typically used when compression affects multiple levels of the cervical spine or when spinal stability needs to be restored. This procedure focuses on relieving pressure on the spinal cord or nerve roots while providing stabilisation through fusion. PCDF may be recommended in cases of cervical spinal stenosis, myelopathy, or complex cervical pathology.

Scoliosis Correction Surgery

Scoliosis correction surgery is used to address abnormal curvature of the spine that may cause pain, imbalance, or neurological symptoms. Surgery aims to correct spinal alignment and stabilise the spine to prevent further progression of the curve. This procedure is typically considered when scoliosis is severe, progressive, or associated with functional or neurological concerns.

How Conventional Spine Surgery Fits Into the Care Pathway

Conventional spine surgery is one part of a broader treatment pathway. Before surgery is considered, patients are usually assessed for non-surgical options such as:
  • Medication and activity modification
  • Physiotherapy or rehabilitation
  • Pain management procedures
Surgery is discussed when these measures are insufficient or when neurological compromise, instability, or deformity is present.

Care at Oxford Spine and Neurosurgery Centre

Spine surgery at Oxford Spine and Neurosurgery Centre is guided by individual assessment and structured treatment planning. Both conventional and minimally invasive approaches are considered based on each patient’s condition and needs. Care is led by Dr Colum Nolan, Senior Consultant Neurosurgeon with more than 20 years of clinical experience, with emphasis on clear communication, appropriate surgical selection, and coordinated follow-up.

In Summary

Conventional spine surgery includes established surgical techniques used to treat complex or advanced spinal conditions. Procedures such as ACDF , PCDF, and scoliosis correction play an important role in relieving spinal cord or nerve compression and restoring stability when less invasive options are not appropriate.
